BY DONATING ITEMS FOR THE RAFFLE AND SILENT AUCTION.

Your donation provides valuable support for Kol Tikvah and the work done in the community.
This year a portion of the proceeds are being donated to the West Valley Food Pantry, a coalition of nine congregations
located in the West San Fernando Valley providing emergency food to the needy and homeless of that area.  Donors will receive acknowledgement in the auction program and on the bidding sheets for items included in the silent auction.
All donations are tax deductible in accordance with applicable laws.

Airplane trip for a day

Themed Gift Baskets/ Bottles of Wine

Handmade Crafts such as Quilts, Jewelry, etc.

In addition to the silent auction we are also looking for raffle items and door prizes.

Complete a separate donation form for each item you are donating and return to Kol Tikvah via Fax
818-348-1584 by  Friday, May 9th  for inclusion in the printed auction program distributed the day of the golf tournament.
Please keep a copy for your records.  You will receive a confirmation notice within 10 business days from receipt of this form.
